High Value Medications Project

PATIENT AND CLINICIAN MATERIALS

Medpin has developed educational materials providing drug information, therapeutic class comparisons, and clinical care guidelines for outpatient care of safety net patients with diabetes and/or high cholesterol.

Treatment of Hyperlipidemia Monograph (pdf)
Overview of treatment of hyperlipidemia with statins , including discussion areas such as mechanism of action, indication, dosage, drug and food interactions, warnings, monitoring and contraindications. (6 pages)

Hyperlipidemia Clinical Care Guideline (pdf)
Treatment guidelines for hyperlipidemia developed for the safety net provider environment. Upon request, laminated versions are available at no charge from Medpin. (2 pages)

Clinician Tip Sheet (pdf)
Summary of key points from the monograph, specifically geared to safety net populations. (2 pages)

Statin Comparison Chart (pdf)
Chart comparing use of statin agents. Due to changing availability of medications in safety net setting, patients may need to be moved to a more available agent. (1 page)

DIABETES

Treatment of Type 2 Diabetes Monograph (pdf)
Overview of treatment of type 2 diabetes. Includes discussion areas such as mechanism of action, indication, dosage, comparison of oral and injectable agents, drug interactions, warnings, monitoring and contraindications (11 pages)

Treatment of Type 2 Diabetes Clinical Care Guideline (pdf)
Treatment guidelines of type 2 diabetes developed for the safety net provider environment. Upon request, laminated versions are available at no charge from Medpin. (4 pages)
